I understand as the undersigned legal parent/guardian of a child who is participating in a Camp at the Calgary Science Centre & Creative Kids Museum Society (hereinafter referred to as "TELUS Spark"), that my child will participate in activities on the grounds of TELUS Spark. I hereby irrevocably, unconditionally and forever waive, release and agree to indemnify and save harmless TELUS Spark and its trustees, officers, directors, employees, volunteers, committee members and agents from all claims, damages, loss and/or injury to my child or my child’s personal property arising from, or in any way resulting from, my child’s participation in these activities. I have provided TELUS Spark with all the necessary medical information required and I can be reached at the number(s) listed. I authorize TELUS Spark to administer first aid to my child and to secure medical care for my child in an emergency situation as deemed appropriate by the attending physician(s). I understand that the personal information collected is used for registration and emergency purposes only. I understand that TELUS Spark videotapes various areas of the facility for security purposes, which may include areas in which organized activities occur. I agree that my child may be videotaped for security purposes only. I certify that the information provided in this registration form is, to my knowledge, true and complete. I understand that this Waiver, Release and Indemnification is governed by the laws of the Province of Alberta. I confirm that I have had sufficient time to read and understand this Agreement in its entirety and that I am the full age of eighteen (18) years. I understand that this Agreement represents the entire Agreement between me and TELUS Spark.
